Personal Home Care Assistance

365 On Call works with private companies that will assist you in personal home care assistance that may provide you with specialized care services which includes: Alzheimer’s, Dementia and Memory Care; Post-hospital care; 24-hour Daily care; Respite care for family caregivers; and companionship.

Community Care Services

Our patient coordinators in the office will assist you in finding the right people to help you with Community Care Services such as: Personal care assistance; Homemaker; Case management; Escort; Sitter services in the hospital.

Durable Medical Equipment (DME)

365 On Call will assist you in evaluating your need for any Durable Medical Equipment such as motorized wheelchair, hospital bed, walkers, canes, etc. We work with trusted professionals that can assist you in getting the needed DME without any hassle.
