Our skilled health care professionals provide the care centered on the individual and the education needed to help with the recovery from an injury, surgery, or illness. Our expert health care team will help these individuals maintain their independence, all while reducing the risk of unplanned hospital admissions. 365 On Call will do this through the following services:

Initial and Follow-up Home Visits

Initial and follow-up home visits by the Physicians and/or non-physician providers i.e. Nurse Practitioners, Physician Assistants. The said providers will initially evaluate your homebound status per CMS guidelines to make sure the services we provide are justified per your health care needs.

On-Call Access

We offer on-call access to our providers (physician and non-physicians) 24 hours a day, 7 days a week to respond to patients with emergency medical consults after work hours to assure continuity of care.

Convenience

365 On Call providers will do the following services at the convenience of your home:

• PHYSICAL EXAMINATION

• ANNUAL WELLNESS EXAM

• DIABETIC ASSESSMENTS

• WOUND CARE
